A few things come to mind off the top of my head:

1. Have you fired many 45LC rounds without a through cleaning?
The shorter 45LC round can leave a "ridge" of lead and powder fouling that make it difficult to insert cartridges fully into the chamber.
Well documented with 454/45LC, 44mag/44 special, 357mag/38 special.

2. New Brass --Resized full length? Trimmed?
Even new cases need to be resized and checked for trim length prior to loading in YOUR pistol. Every set of chambers is unique.

3. Overall length of cartridge
Possible that the bullet is seated too far out where the ogive begins to seat against the taper of the cylinder.

4. Oversize slug? 0.457-459 inch Black powder or rifle bullet used??

I had a similar problem. I finally figured out the bullet seating die was improperly adjusted and was causing the case to bulge just slightly I couldn't see it just by looking at it but I checked measurements and seen a bulge toward the head of the case.

I would only add one small thing besure if you you are using cast lead bullets that you are only shoving the bullet down until the top of the brass is able to be crimped into the cannnela(crimpgrove) . If not then the brass will be swelled at the top.

some 454's iv'e seen the forcing cone started in the chamber
you may need to seat the bullets a little deeper in the case..

What dies do you have? I found the same problem with my Lee dies. I ended up ordering a separate crimping die, which solved the problem. The ammo just falls right into the chamber now. The seating dies in the standard set didn't seem to have a crimping feature on the die.

I just bought .357 mag Lee carbide dies and did not think the bullet seater crimped either until I screwed it about 1 turn further into the press than the instructions directed. Once I did that, it looked like a made a factory bullet!
